Built in 1371, the church kept the relics of San Clemente and Santa Vittoria (now in the parish church), kept in a gilded wooden urn equipped with four locks whose keys were each entrusted to the parish priest of San Giovanni, to the chaplain of the church of the Madonna delle Grazie, to the representative of the local community and to a member of the Poor Hospital of Pian di Carpine, run by the Basilian monks, originally annexed to the church.
The fresco depicting the Madonna nursing the Child, attributed to Bartolo di Fredi and Jacopo di Mino del Pellicciaio, was probably on the outer wall of the hospital.
